The Initial Visit
Our compassionate clinical team will do whatever it takes to make every visit to our office a pleasant, relaxed experience. With all of the important information about our practice available on our website, you can feel confident that you and child are well-prepared for your first appointment. To expedite your check-in process, please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
Additionally, please register the patient on the Patient Portal: https://12077.portal.athenahea... . This fantastic feature of our electronic health record system allows you to:
- View and print Immunization Records
- See visit notes from your doctor
- View lab results
- Make appointments
- Leave an electronic message for your doctor
- Print forms and e-sign any documents necessary for your appointment
Patient Forms
Patient Forms are available on the Patient Portal and can be accessed under 'My Health>Medical Forms". Questions about which forms you need to review or sign? Please call our office and our courteous staff will assist you in preparing for your child's first visit.
Directions
San Dimas: We are located next to San Dimas Community Hospital in the 1330 Building. Our office is on the first floor.
South Pasadena: We are located on Huntington Drive, at the intersection with Fletcher Ave. We are located on the first floor, suite C. Parking is available behind the building on Fletcher Avenue.
